Jurassic Coast Primary Care NetworkWe are looking for an Advanced Clinical Practitioner (Nurse or AHP) who has completed an Advanced Practice programme (minimum 120 credits at Master’s level) or who is currently on a Master’s programme of study for Advanced Practice. The applicant must demonstrate excellent clinical, communication and organisational skills and enjoy working in an established, innovative Advanced Practitioner led team. A clinical background in older person’s care and frailty is important. The role will involve performing same-day home visits to frail patients living at home or in care homes across the PCN. You will gain a systematic patient history, perform assessment and physical examination and diagnose before making a management and treatment plan in partnership with the patient and the MDT. The immediate team is comprised of Advanced ClinicalPractitioners, Integrated Care Nurses, Health
Care Assistants and Administrators. The role involves close working within the multi-disciplinary team within primary care, community services and acute hospitals.The catchment area for the primary care network is Bridport, Beaminster, Maiden Newton, Lyme Regis and Charmouth.
