The post holder will be a key member of our Urgent Community Response multidisciplinary team including a Consultant Geriatrician, Advanced Nurse Practitioners, Registered General Nurses, Registered Mental Health Nurses, Paramedic Practitioners, Healthcare Assistants and Therapists to provide a 2-hour response to patients who are experiencing a health crisis with the aim to support them in their own homes (including care homes) rather than being admitted to hospital.
You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will receive ongoing clinical support, education, and supervision.
The post holder will be required to liaise with their team colleagues in the other localities covered by the service to ensure cross cover can be provided according to demands on the service.
The Urgent Community Response Service is a 7-day service and operates from 08.00 – 20.00hrs. You would be expected to be able to cover all aspects of the shift pattern.
