To undertake various clerical duties associated with the custody and use of Health Records. This to include all administrative procedures and documentation associated with a patient’s admission and/or attendance to the Trust, as outlined in the Patient Access Policy.
It is expected that staff will undertake duties relating to any aspect of service provided by the Health Records Team and at any of the Departments in The Royal Wolverhampton Hospitals NHS Trust.
It is expected that staff will be able to demonstrate a significant level of initiative in organising their workload and in taking decisions and action within clearly defined limits.
To ensure patient confidentiality is maintained at all times.
To prepare case notes for all attendances ensuring they comply with the Standard Order of Case Notes, and to ensure X-ray and investigation reports relevant to each attendance are available.
To accurately in-put, up-date and monitor patient related information onto the computerised Patient Administration System.
To be responsible for the security of case notes in the department.
To ensure the timely delivery of prepared clinics to the appropriate Out-Patient Departments.
To work closely with Medical Secretarial staff, to ensure the development of an efficient team to effectively support the Directorate.
To arrange transport for patients as required.
To amalgamate case notes where more than one record exists and to ensure that all casenotes are filed in the recognised format.
To take action as necessary, to book, cancel, change or expedite appointments on the Patient Administration System.
To be responsible for the timely processing of all incoming and outgoing mail

To liaise closely with the Central Services Team on all matters affecting the efficient production of case notes.
To collect relevant statistical data relating to patient activity.
To be actively involved with procedural changes which may arise and to provide information/case notes as requested by Medical Staff for such purposes.
To assist with the training of newly recruited health records staff, including an introduction into working practices and procedures within the Directorate/Central Services Team.
To be aware of the security arrangements in your own working area and to be responsible for the security/distribution of keys held in the department.
To cover in the absence of colleagues as required.
To attend mandatory training sessions and undergo training as deemed appropriate by Managers.
To take part in the annual appraisal system.
To be familiar with and comply with all Directorate and Trust policies, procedures and practices.
To undertake any tasks relevant to the work of a Health Records Clerk which develop as a special requirement of the Directorate/Department.
To provide a patient focused reception service, ensuring that the service is efficient and welcoming.
To undertake the retrieval of case notes/ x-rays as required.
To accurately file case notes returned to the Department in a timely manner.
