Frimley Integrated Care System is transforming community mental health services for people with significant mental health problems. A ground-breaking new model is being implemented within Berkshire Healthcare NHS Foundation Trust and Surrey and Borders Partnership NHS Foundation Trust to benefit those patients within the primary care setting.
We are seeking to recruit experienced mental health practitioners to lead teams in our Primary Care Networks in Bracknell. The post holder will be responsible for working closely with General Practitioners and the primary care team to provide asset-based assessments, brief interventions and care navigation for people with significant mental health issues living in the locality.
