An exciting new role to
support Primary Care Networks and Primary Care services in Bracknell & Ascot.
Berkshire Primary Care Ltd (BPC) is a GP Federation providing NHS services for
all 12 practices and 3 Primary Care Networks (PCNs) in Bracknell and Ascot. We
serve a population of 143,000 patients and have been chosen by Bracknell &
District and Ascot PCNs to be Employers for the Primary Care Network additional
roles.

The role will involve a
mix of working in our centralised Pharmacy Hub and in a more patient facing role in
Practice, playing an important role
complementing the clinical pharmacists and other members of the
multi-disciplinary team. Initially focusing on supporting the Primary Care
Network to implement medicines optimisation requirements; the delivery of safe,
effective and efficient systems for repeat prescribing; reducing waste and
ensuring effective and efficient use of medicines through shared
decision-making conversations with residents/patients. As the PCN matures, the
focus of the role will expand to meet the requirements of the new Network
Contract Direct Enhanced Services responsibilities
