Community Health NHS Trust (rated Good by CQC) It is an exciting time to join our diverse and dynamic team, who
have joined forces with Shropshire and Telford Health and Social Care team to
become one Integrated Discharge Team all based together in Royal Shrewsbury
Trust.

The Patient flow
Co-Ordinator will be responsible for the scheduling of admissions to our community
hospitals and independent sector beds and provide daily bed capacity and flow
reports. They will be required to achieve optimum
efficiency of the use of all beds and ensure that admissions are effectively
coordinated and arranged.37.5 hours
