We have an
exciting opportunity for two enthusiastic Paramedics to join our dynamic team.
This permanent position is well suited to individuals who are enthusiastic
about providing high quality support to primary care.Based within
Medway Peninsula PCN, you will work closely with GP Practices and Care Homes
within the primary care network. GP Practices include Highparks Medical
Practice, The Elms Medical Practice and St Werburgh Medical Practice in
Rochester, Kent. You will be
joining forces with our established paramedic and a team of clinicians within
the PCN, This includes Pharmacists, First Contact Physiotherapists, Physicians
Associates, and Social Prescribers.

Although your
working days and hours will be standard, your work patterns are likely to vary
and may consist of home visiting as well as providing clinics in GP practices
seeing patients with a variety of conditions. Your allocation of patients will
be according to your clinical skill set/competencies and following a triage
process to ensure patients are allocated to the appropriate clinician.
