A fantastic opportunity has arisen for a Band 6 Occupational
Therapist to join our South East Shropshire Community Therapy Team based in
Bridgnorth.The team is multidisciplinary and comprises of registered
Physiotherapists, Occupational Therapists, Support Workers and Administration
Workers.The team provides a seven-day service across Southeast
Shropshire.The team provides support and rehabilitation to patients with a
range diagnosis and complex conditions within the community setting.The teams have established links with key stakeholders, and
wider support services within the county. The team also work closely with
community nursing and social care to support transitions within the patients
care pathways and enabling coordinated joint working for patients with complex
needs.Community based rehabilitation aligns with the Shropshire
Telford & Wrekin Integrated Care System vision of empowering people to live
their best life, promoting self- care and independence.We expect the very best from our staff in return we can offer
