Working with the Rough Sleeper coordinator the post holder will provide advice and support into the rough sleeper task force. Working under the supervision of a registered nurse to provide high quality care and treatment as a member of the multi- disciplinary/ multi agency team to support the rapid assessment and support to adults 18+ who are homeless or rough sleeping. This may include undertaking a full range of routine care and treatment tasks, as directed by the rough sleeper coordinator, within community settings with patients who are experiencing mental health conditions. This will include undertaking observations and reporting on patients’ conditions.
To provide specialist mental health support to adults who are housed and supported by the local authority but whose tenancy is in jeopardy due to a decline in their mental health as directed.
