This post offers a unique
opportunity to work in an integrated way with primary and community care,
focusing on the management of patients with frailty. Working as part of the
Primary Care Network Frailty Team the postholder will support the visiting
requirements and use their acute clinical skills to support patients.The postholder will work closely
with GPs within the growing PCN frailty MDT, which includes GPs, Advanced
Practitioners, specialist nurses, specialist care home pharmacists, care coordinators
and social prescribers, and our partners in community nursing and therapy teams
