An exciting opportunity has arisen for new Community Rehabilitation Assistants to join Poole Intermediate Care Service.
The chosen candidates will work as part of a Multi-disciplinary Team caring for patients with an illness in their own homes to prevent hospital admissions and also to facilitate complex discharges from hospital. You will work alongside qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians and wider locality teams. Good communication skills and a caring attitude are needed.
Shift working is required as the service operates 365 days a year 08.00 to 20.00. The successful candidates will need to work some weekends. Full or part time permanent hours are available. There is a need to use your car to visit patients in their own homes. Car owner/driver essential subject to the provisions of the Equality Act 2010.
Basic IT skills and NVQ level 2/3 would be advantageous.
