We have a fantastic opportunity to recruit an experienced Nurse for an Advance Nurse Practioner role to join the Wokingham Urgent Community Response Team. The role will provide advanced clinical assessment skills to patients in their own home and in a care home, who require an urgent response to prevent unnecessary admission to hospital. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care.The role will provide leadership and line management to junior staff within the multidisciplinary team, including nurses, therapists and support workers, and clinical supervision and training/ education to the nurses and support workers, as required. You will be a team player and work collaboratively with staff from all disciplines whilst maintaining
high professional standards. You will ensure good communication, and that effective processes and procedures are maintained across the service. The post holder will work 37.5hrs per week, 7 days a week, between the hours of 8am – 8pm, which will include weekends.The role is based at The Old Forge Wokingham and staff parking is available locally.
