The practice has a loyal group of patients from a tightly knit community. We look after two care homes and the practice has a slightly higher proportion of older patients than the national average.The practice is seeking to move from a traditional model of general practice to a more multi- disciplinary model and we are keen to recruit one or more Advanced Care Practitioners to lead this change. The ACP(s) will play a key role in moulding the current GP, nurse, paramedic and pharmacy team into a new model of clinical care that reflects the needs of the practice and establishes a cutting edge approach to primary care. A strong, well established management/administration team stands ready to support this change.The ideal candidate will be an independent non-medical
